After 5 years of servicing our community with their medical imaging needs, we are saddened to inform you that we closed the imaging center on December 31, 2011. Your medical records are secured with an NMI Custodian.

You will need to complete an authorization form to release your medical records. Please select the image to the left and send your request to NMI Custodian at the following address:
